Permission Slip and Health Form
for Troop 33 Activities
The following is a link to the new combined permission slip/health form that is used for Troop 33. Each Scout must have this form filled out prior to attending a Troop 33 campout. Any guest/sibling joining Troop 33 for a campout must fill out the form as well.
Parents who would like to go camping with Troop 33 are encouraged to read the attached Parent Guidelines document.
